About the role
Deliver virtual chronic care support for Medicare patients enrolled in Cadence care programs. Provide coaching, care coordination, and patient outreach to improve health outcomes.
What you’ll do
• Conduct outbound telephonic patient outreach and coaching. • Help patients execute personalized care plans and goals. • Conduct comprehensive health and lifestyle assessments. • Educate patients on disease management and medication adherence. • Document interactions and collaborate with clinical teams.
What you’ll bring
• Active NBHWC health coaching certification. • At least 1 year experience as a health coach. • Experience supporting chronic condition patients. • Fluency in Spanish required. • Remote work readiness with stable high-speed WiFi.
