About the role
Provide virtual chronic care support for Medicare patients enrolled in Cadence care programs. Deliver patient-centered outreach, coaching, care coordination, and documentation to support chronic disease management.
What you’ll do
• Conduct outbound telephonic patient outreach and coaching. • Help patients execute personalized care plans and goals. • Conduct comprehensive health and lifestyle assessments. • Educate patients on disease management and medication adherence. • Document interactions and collaborate with clinical teams.
What you’ll bring
• Active multi-state LPN/LVN license required. • At least 5 years clinical experience as Licensed Practical Nurse. • Fluency in Spanish required. • Remote work readiness with stable high-speed WiFi. • Experience with AI-assisted clinical tools preferred.
