About the role
Provide comprehensive case management services to frail elders and adults with disabilities in the Wisconsin Family Care program. Coordinate care plans and support services to ensure member health and safety.
What you’ll do
• Assess members' health and safety needs. • Coordinate with Field Care Nurse for service delivery. • Conduct face-to-face social assessments. • Arrange support services for social integration. • Maintain accurate documentation and case notes.
What you’ll bring
• Bachelor's degree in human services or related field. • 1+ year experience with frail elders or adults with disabilities. • Valid state driver's license and vehicle insurance. • Reside within 45 minutes of coverage area preferred. • Experience with case management and electronic documentation preferred.
