About the role
Provide integrated support to patients and physicians to improve medication adherence and promote evidence-based care. Develop personalized relationships with patients and prescribers to enhance quality of patient care.
What you’ll do
• Review clinician's patient care notes for prior authorization approvals. • Develop clinical protocols to maximize patient adherence. • Monitor patient adherence across disease categories. • Evaluate EMR for medication and therapy optimization. • Supervise support personnel and ensure compliance with regulations.
What you’ll bring
• Doctor of Pharmacy (PharmD) or equivalent degree. • Licensed Pharmacist in Oregon or commit to licensure. • 1+ years experience preferred. • Strong verbal and written communication skills. • Proficient in Microsoft Office and database tools.
