About the role
Serve as a trusted frontline public health worker partnering with patients and community organizations to improve access to care and support chronic disease self-management. Provide health system navigation, resource coordination, outreach, education, and culturally responsive coaching without clinical assessment or treatment.

• Partner with patients, families, and primary care teams. • Improve access to care and address social drivers of health. • Support chronic disease self-management. • Coordinate resources and provide outreach and education. • Reinforce care plans and provide culturally responsive coaching.
What you’ll bring
• High School Diploma or GED. • Experience in healthcare or community-based services. • Knowledge of local community resources. • Completion or willingness to complete CHW training. • MiCHWA CHW certification preferred.
