About the role
The Community Health Worker will serve as the frontline public health worker for patients in the community. They will support healthier behaviors and link people to healthcare and social services.
What you’ll do
• Build understanding and social capital for healthier behaviors. • Provide informal counseling and follow-up with patients. • Link people to healthcare and social service resources. • Deliver health information using culturally appropriate terms. • Help people understand health conditions and improve well-being.
What you’ll bring
• Understanding of healthcare system and patient resources. • HND, NCE, or OND qualification. • Experience with diverse cultures and socio-economic backgrounds.
