About the role
Provide virtual care and supervision for Nurse Practitioners in a whole-person care model. Support Medicare patients with complex medical, behavioral, and social needs.
What you’ll do
• Collaborate with Nurse Practitioners using clinical judgment. • Develop and refine clinical programs. • Support care coordination for complex patients. • Work remotely with provided equipment. • Engage in interdisciplinary team collaboration.
What you’ll bring
• 2+ years clinical experience with complex patients. • Board certified in Family Medicine. • Licensed in CA plus 2+ additional states. • Experience with Medicaid/FQHC and Medicare patients. • Technology-savvy and telemedicine comfortable.
