About the role
Manage individualized case navigation for approximately 400 clients at community service centres. Support treatment continuity and link clients to relevant health services.
What you’ll do
• Maintain reconciled client list and follow-up arrangements. • Support treatment continuity and appointment attendance. • Link clients to HIV, TB, maternal and child health services. • Coordinate community index-testing and outreach. • Track referrals and document outcomes.
What you’ll bring
• SSCE or equivalent with 3 years relevant experience. • Or health/social-service diploma with 1 year experience. • Confidential and non-discriminatory engagement. • Local language and catchment knowledge desirable. • Ability to work collaboratively with teams.
