About the role
Assist in developing and coordinating holistic care management for long-term care members. Educate members and caregivers on available services and benefits.

What you’ll bring
• Bachelor's degree or equivalent experience. • 1 year related experience. • Experience with individuals with disabilities or vulnerable populations. • Knowledge of chronic or complex conditions. • Must reside in Texas.
